Catherine Hustad, MSN, RN, FNP-C

Catherine Hustad is a Family Nurse Practitioner (FNP) specializing in Primary Care. Prior to becoming an FNP in 2016, she obtained RN experience in home care and hospital settings. She also worked for many years in the Emergency Department. Catherine completed both her BS and MS degrees in Nursing at Southern Adventist University in Collegedale, Tennessee, and holds National Certification with the American Academy of Nurse Practitioners. Catherine is passionate about lifestyle medicine and helping her patients achieve optimal wellness.
Jason Holritz, FNP-C

Jason Holritz is an FNP seeing patients with both acute and chronic conditions. As well as becoming an FNP in 2017, Jason holds his master's degree in Theology and has earned his National Certification with AANP. He has worked in ministry and higher education and is very passionate about educating his patients to better control their health and wellness. He continuously dedicates himself to their well-being every day.
Grace Adeniji-Ileseanmi, FNP-BC

Grace is a Board-Certified Family Nurse Practitioner with sixteen years of experience in the medical profession. Grace received her Bachelor's and Master of Science degrees in Nursing from the University of Tennessee at Chattanooga in 2005 and 2012, respectively. Her scope of practice encompasses disease prevention, health coaching, and management of acute and chronic health problems. Grace's philosophy of practice is empowering the patient through education and motivation to live a healthy lifestyle.
Anna Borneman, MSN, FNP-C

Anna is a certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. She obtained her BS and MS degrees from Lincoln Memorial University in Knoxville, TN. Before becoming a Nurse Practitioner, Anna worked in a cardiac setting at a local hospital for many years. She treats acute and chronic conditions and enjoys building relationships with her patients. Anna strives to help her patients achieve their optimal wellness goals.
Stefanie Swingle, MSN, FNP-C

Stefanie is a Board-Certified Family Nurse Practitioner with 28 years of experience in the medical profession. She received her Bachelor of Science in Nursing from Carson Newman College in 1996. She then worked in several different hospital settings that include the Neonatal ICU, Cardiac ICU, Medical/Surgical ICU and Emergency Medicine. Stefanie furthered her education and graduated with her Master of Science in Nursing from Vanderbilt University, Nashville, Tennessee in 2010. She worked as a Neonatal Nurse Practitioner and then returned to earn a Postgraduate degree as a Family Nurse Practitioner from Southern Adventist University and holds National Certification with the American Academy of Nurse Practitioners. Stefanie has experience as a Practitioner in Dermatology, Plastic Surgery, Acute Care, Urgent Care, Gastroenterology, Internal Medicine and Family Practice. Stefanie strives to deliver a holistic approach to medicine and provide excellent care to her patients physically, emotionally and mentally.
Ann Geraci, FNP

Ann has been a Family Nurse Practitioner for more than 14 years and in the medical field for almost 20 years. She received her Bachelor of Science in Nursing from UTC’s nursing school in 2003 and began working in emergency medicine and cardiac rehab. She furthered her education and graduated with her Masters of Science in Nursing from Southern Adventist University in 2008. She is certified by the American Nursing Credentialing Center. Her patient experience ranges from infants to the elderly. She loves getting to know her patients, developing individualized treatment plans, and having the opportunity to listen to their rich history.
Heather Mayo, MSN, FNP-C

Heather received her BSN degree from the University of Arkansas and her MSN degree from the University of Cincinnati. She is certified in Family Practice, and prior to One to One Health she worked for the Health Department in Family Planning.
Brandi Serret, CMA

Brandi has worked as a medical assistant for over 8 years and spent 4 years working with the Chattanooga Heart Institute. She received her certified medical assistant degree from Chattanooga State Community College and is working towards her RN degree. Brandi is passionate about patient care and enjoys building relationships with her patients.
Faith Hunt, CMA

Faith is a CCMA and a Certified Health Coach. She graduated from Academy of Allied Health in 2014. Over the last 16 years she has worked in pediatrics all the way through geriatrics. She’s worked in specialty offices of Cardiac, ENT and Endo. Her passion is working with bariatric patients. Her goal is to be a nurse practitioner as she loves to build relationships with her patients and to encourage them.

Wendi is a licensed practical nurse and a 2003 Chattanooga State Community College graduate. She has over fifteen years of experience working in the medical field, and her work history includes pediatrics, allergy and immunology, and cardiology. Wendi is on her way to completing her RN and BSN degrees.
